As a Concentra patient, you will receive a complete evaluation from an experienced therapist. Your therapist, working with your physician, will then create a customized recovery plan to speed healing and help provide ways to prevent your injury from recurring.
- Doctor's referral — Once your physician refers you to Concentra, you simply call us toll free at 1-888-997-2669 to schedule an appointment. In most cases, you'll begin treatment with one of our licensed therapists within 48 hours. A copy of your signed prescription and proof of coverage by a participating private health insurance plan are all you need. To better serve you, we accept over 170 insurance companies and PPO networks including Aetna, Cigna HealthCare, PHCS, Humana, Blue Cross/Blue Shield, and MultiPlan.
- Finding us — We have 319 locations that are strategically located in metropolitan areas throughout the nation. Use our Location Finder for information and map the closest Concentra Medical Center to your work or home.
- Key papers — When you arrive at the medical center, you'll need to provide our staff a copy of your signed prescription and insurance card. (See Doctor's referral paragraph above for information about participating insurance plans.)
- Meeting the therapist— After reviewing your doctor's diagnosis and notes, our physical therapist will greet you and ask general questions about your pain, your physical history, and your overall health. The therapist will then begin a comprehensive initial evaluation.
- Developing your customized treatment plan — Because every patient and every injury is different, our therapist will create a specific exercise regime for you. This will be based on your doctor's diagnosis as well as your physical condition, past injuries, age, etc. Following this plan will be your key to getting better faster.
- Creating your Home Recovery Plan— Physical therapy is hard work, but the more effort you put into it, the faster you will get better. The therapist will work with you in the center and instruct you on how to do the exercises properly. However, the more you do at home, the right way, the faster you will feel better.
